This form must be completed & signed by homeowners.
If a homeowner will be having someone else fill out the cabin prep form for them (such as a family member, caretaker or management company),
we will need this CONSENT FORM filled out and returned stating that you give this individual permission to provide information on your behalf. 

Homeowners MUST read, understand & agree to ALL terms and conditions if we will be opening and/or closing their home. TERMS & CONDITIONS

The information on our cabin prep form below will be used at all home openings/closings so please be sure everything included. *Even if you think we already have or know it.
EP cannot be held responsible for incomplete or incorrect information.  
Please do not put information on form that does not pertain to openings/closings.
