This form must be completed & signed by the legal homeowner.
If someone else will be filling out the form for the homeowner (such as a family member, caretaker or management company), the legal homeowner is required to fill out and return this CONSENT FORM stating that they give this individual permission to provide information on their behalf. 

Homeowners MUST READ, UNDERSTAND, & AGREE to ALL terms & conditions PRIOR to signing form.  TERMS & CONDITIONS

The cabin prep form below will be used at ALL OPENINGS & CLOSINGS moving forward.  It is the homeowner's responsibility to be sure we have the most current/correct information for their home. This form will ONLY need updated if the information provided changes.  If you need to make changes, please submit a new form using the link at the top of our website (do not edit old form). Please DO NOT answer questions with "you already have/know this" OR "same as always", etc... We would be happy to provide you a copy of your form once it is submitted, upon your request.
EP cannot be held responsible for incomplete or incorrect information provided by homeowners.

DO NOT PUT DATE REQUESTS ON FORM, or information that does not pertain to the questions asked/task at hand.
