This form must be completed & signed by the legal homeowner.
If someone else will be filling out the form for the homeowner, such as a family member, caretaker or management company, we will need the homeowner to fill out and return this CONSENT FORM stating that they give this individual permission to provide information on their behalf. 

Homeowners MUST read, understand & agree to ALL TERMS & CONDITIONS

The cabin prep form below will be used at all home openings/closings, so it is important that ALL information is included.
DO NOT answer questions with "you already have/know this" OR "same as always", etc... 
EP cannot be held responsible for incomplete or incorrect information
 
This form will need updated if the information provided changes. *We can return a copy of the form to you for your reference, upon request
It is the homeowner's responsibility to be sure we have the most current information needed. 
DO NOT put dates on form, or information that does not pertain to the questions asked/task at hand.
