THIS FORM MUST BE COMPLETED BY THE LEGAL HOMEOWNER.
If a homeowner will be having another party fill out the form for them (such as a family member, caretaker or management company), the legal homeowner must fill out and return this CONSENT FORM stating that they give this individual permission to provide information on their behalf. 
 A SEPARATE FORM IS REQUIRED FOR MULTIPLE HOMES, GUEST COTTAGES OR GARAGE APARTMENTS.

HOMEOWNERS MUST READ, UNDERSTAND, & AGREE to ALL TERMS & CONDITIONS PRIOR TO SIGNING FORM.  TERMS & CONDITIONS

The cabin prep form will be used at all openings/closings moving forward and will only need updated if the information provided changes.  It is the homeowner's responsibility to be sure we have the most current/correct information for their home. If you need to make changes a new form must be submitted using the link at the top of our website (do not edit old form) as forms are dated when submitted for liability purposes. 
EP CAN NOT BE HELD RESPONSIBLE FOR INCOMPETE OR INCORRECT INFORMATION PROVIDED BY HOMEOWNERS.

DO NOT PUT DATE REQUESTS ON FORM, OR INFORMATION THAT DOES NOT PERTAIN TO THE QUESTION ASKED/TASK AT HAND. 
 Date requests MUST be submitted IN WRITING VIA TEXT OR EMAIL ONLY.  DO NOT put service requests on form that are not to be done on a yearly basis.  All one-time service requests must be submitted by phone, email, text, OR by submitting a SERVICE REQUEST FORM
